It’s all about population size. If a village grows past 5000 odd residents it’ll become a candidate for “shirehood” replacing the muhtaris with a mayor (kinotarhis or dimarhos).

Oh, and your spelling of “Koinotarchis” is straight out of a uneducated Greek school that cannot comprehend that English pronunciation of Greek words cannot be spelled in Greek for obvious reasons!

The correct spelling if the aim is to assist non-Greek speaking people is “Kinotarhis”.

There you go... see how any Brit can now read and pronounce that correctly?

what ever the titles might in Greek we have two blokes, one of whom is described as Mayor (or Δήμαρχος? ) and another who counter signs some papers, who we always knew at the Mukhtar : if his correct tittle is Κοινοτάρχης, I am happy to stand corrected though that well known translation service Google, translates Κοινοτάρχης as Mukhtar, suggesting some inter-changeability in the terms,
