You would need to explain why you need this (I don't mean your medical condition) and where you are resident to get any useful information about where to obtain one.

oooh , i know where to get it from,
go to the nearest general hospital, ask about the midical card document, fill it in, pay about 10 to 14 euros and you get it in two weeks...
my question is, by any chance can it be useful for someone who wanna have a face left or tummy tuck or any cosmatic surgery for that matter.. hmm that would be good wouldnt it..?

Are you a Cyprus national who is planning to travel to another EU country? If not, you get it from the authorities in your own country (to the best of my knowledge).
