You need to speak to a vet ... at least ~ 7 months before you are planning to make the trip.
The dog has to have a microchip. Then a rabies vaccine. Then one month later a blood test to check he has acquired immunity. Then you can travel to Cyprus 6 months after the blood test was done. But you still need a vet to check the dog and give anti-worm stuff before the flight (24-48 hours).
The vet prepares and fills in the passport.
